Postpartum Weight Loss: Overcoming Thyroid Challenges

Putting women on thyroid medication during IVF seems increasingly common these days. But what is NOT common is getting them off that medication or rebalancing their hormones once a baby has been born or IVF has been stopped. The result, a chronic disease which requires lifelong medication and in many cases sticky postpartum weight gain, which refuses to shed. R began hormonal medication in 2020 when she and her husband were trying to conceive and began IVF treatment. They welcomed a beautiful baby girl in December 2023. 
However, even one year after childbirth—and despite regular breastfeeding—R had not lost her postpartum/pregnancy weight (over 15 kg). Her periods had not resumed even 11 months post-delivery, which some medical professionals consider within the realm of normal. She had also been on thyroid medication (thyroxine, 50–75 mcg) for the past four years.

Since she was breastfeeding and did not want to make drastic dietary changes, we began by modifying just one meal per day in the first month. Gradually moved to two modified meals from the second month onward. Her thyroid medication was tapered gradually. From 75 mcg to 50 mcg, then to 25 mcg, and finally stopped altogether after two months.

Within the first month of treatment, her periods resumed. After 2.5 months, she had lost 9.5 kg. “I can fit back into my old clothes!” she said cheerfully one day.
Her energy levels are palpably high and sugar levels have stabilised. She used to have intense craving for sweet foods late at night before we started.
The stiffness in her legs, thighs, and knees—which she had been experiencing for months prior—had significantly improved. She hardly noticed it anymore. Not only had her periods resumed, but they had also become pain-free—something she hadn’t experienced in years, as she used to rely on painkillers before pregnancy.

We repeated her thyroid tests on march 12, 2025—one month after stopping all medication and on july 6, 2025, 4 months post stopping medication
